  • This week’s idea is closely related to Goodhart’s law: when a measure becomes a target, it stops functioning well as a measure. In healthcare and other complex systems, people can improve the number while weakening the real work underneath it. That is why strong leaders do not ask only, “Did the metric improve?” They also ask, “Did care, judgment, coordination, or patient understanding improve too?”

  • Improvement science also warns leaders to watch for balancing measures. A main metric may improve while another part of the system gets worse. In practice, teams are encouraged to use a family of measures, including outcome, process, and balancing measures, so they can see unintended consequences early instead of rewarding them by accident. 

  • This matters in hospitals because measured improvement is not always the same as meaningful improvement. Research and commentary in healthcare quality have repeatedly pointed to unintended consequences, distorted incentives, and the danger of over-weighting visible targets. The core leadership lesson is simple: metrics are useful signals, but they are not the mission.
